AGREEMENT AND RELEASE OF LIABILITY

To be completed by all clients or other individuals wishing to participate in event hosted by East Coast AP Marketing LLC.

Release of Liability

I, ________________________________________, do hereby waive, release, and forever discharge East Coast AP Markeing., and its shareholders, directors, officers, employees, agents, servants, and instructors (hereinafter “Releasees”) from any and all responsibility for or liability for injuries or damages resulting from my participation in any activities I do also hereby release Releasees from any responsibility or liability for any damage or injury to myself, including, but not limited to, those caused by negligent or grossly negligent acts of omission or commission by them, or in any way arising out of or connected with my participation in the activities hosted by East Coast AP Marketing.

Assumption of Risk

. I accept that risk and acknowledge that I am voluntarily participating in these activities hosted by East Coast AP Marketing. I hereby assume and accept any and all risks of injury or death, and therefore release Releasees of any responsibility or liability for any such event.
